Programs

Home and Community-Based Services (HCS)

Vita-Living, Inc. has been a provider of Home and Community-Based Services since 1987. Currently individuals are supported in 15 group homes settings, 25 foster/companion care homes or with supported home living services in their own homes. Individuals served in this program must have a primary diagnosis of mental retardation or other related condition and be eligible for Medicaid. The main emphasis of the program is to help people to become and remain as independent as possible and to be involved in their community.

HCS provides Residential Support, Day Habilitation, Nursing, Dental and Behavioral services, Speech, Occupational and Physical Therapies, Minor Home Modifications, Adaptive Aids, Respite and Case Management.

HCS is a "first come first served" program with individuals taken from the statewide interest list. Initial enrolments are completed through the individuals local MHMR Center. Counties served are Austin, Colorado, Fort Bend, Harris, Matagorda, Waller and Wharton.

Intermediate Care Facilities Serving Persons with Mental Retardation (ICF/MR)

Vita-Living, Inc. began operating in 1982 serving individuals in the Beechnut house. There are currently three ICF/MR homes serving six individuals each. Individuals served in the program must have a primary diagnosis of mental retardation or related condition and be eligible for Medicaid. Admissions are dependent upon bed availability. The program emphasizes positive behaviors in a homelike setting for adults who have severe and moderate levels of mental retardation. There is an emphasis on developing self-help, safety and appropriate social skills.

Community Living Assistance and Support Services (CLASS)

Vita-Living, Inc. has been an independent CLASS case management agency since 1991 and delivers only case management services. A separate direct service agency delivers all other CLASS services. It is believed that under this independent case management model, case managers can better advocate for and focus on participants’ interests. CLASS is a “first come first served” Medicaid waiver program and participants are taken from a statewide interest list. Participants must be eligible for Supplemental Security Income (SSI) or Medicaid benefits and have been diagnosed with a “related condition”. Related conditions include cerebral palsy, epilepsy, autism, spina bifida, head and spinal cord injuries, muscular dystrophy and multiple sclerosis. These conditions must occur before the participants 22nd birthday and considered likely to continue indefinitely. The case manager and the direct services agency work together with the participant as a team to develop an individualized plan of services. Services included in the waiver are: case management, habilitation, nursing, respite, physical, occupational and speech therapies, psychological services, adaptive aids and minor home modifications. Counties served are Austin, Brazoria, Colorado, Fort Bend, Galveston, Harris, Matagorda, Montgomery, Waller and Wharton counties.
